What causes skin sensitivity reactions to ingredients?

Skin reactions to ingredients typically fall into three categories: irritant contact dermatitis (a non-immune response to direct irritation), allergic contact dermatitis (an immune-mediated response to a specific sensitizer), and pseudo-allergic reactions. Fragrances and preservatives are among the most common causes of allergic contact dermatitis from cosmetics. Irritant reactions are more common with acids, high-alcohol products, and physical abrasives.

Are fragrance-free products better for sensitive skin?

For individuals with known fragrance sensitivity or allergic contact dermatitis to fragrances, fragrance-free products reduce exposure to common sensitizers. The EU requires mandatory labeling of 26 fragrance allergens above 0.01% in leave-on products. Note that "fragrance-free" and "unscented" are different: unscented products may contain masking fragrances. Fragrance-free is the more protective designation.

Is niacinamide suitable for sensitive skin?

Clinical data generally shows niacinamide to be well-tolerated across skin types, including sensitive skin, at concentrations up to 5%. Studies suggest it may help strengthen the skin barrier (increasing ceramide and fatty acid synthesis), which can reduce sensitivity over time. A small percentage of users report a "flushing" sensation, which is typically associated with niacin contamination rather than niacinamide itself.
