What causes skin sensitivity reactions to ingredients?

Fragrances and preservatives are among the most common causes of allergic contact dermatitis from cosmetics. Are fragrance-free products better for sensitive skin?
For individuals with known fragrance sensitivity or allergic contact dermatitis to fragrances, fragrance-free products reduce exposure to common sensitizers. The EU requires mandatory labeling of 26 fragrance allergens above 0.01% in leave-on products. Note that "fragrance-free" and "unscented" are different: unscented products may contain masking fragrances. Fragrance-free is the more protective designation.
Is niacinamide suitable for sensitive skin?
